Wellnet is different from Indwell's other brands. It does not run all year. It begins when something happens: a fall, an emergency room visit, a hospital stay. A care coordinator and a community health navigator step in, learn what the person needs, and line up the right help around them.
Wellnet is built to work alongside hospital discharge planners and social workers, who are often the first to know that a family needs help.
The first days after a hospital stay are when recoveries most often go wrong. Medications change, the home is not ready, and no one is sure who to call. Transitional care models, including the Naylor Transitional Care Model and the Coleman Care Transitions Intervention, have found that a coordinated handoff from hospital to home reduces return trips to the hospital.
Wellnet brings that idea to a small community, where the coordinator and the family know each other by name.
Wellnet pairs a credentialed clinical lead with a community health navigator. The clinician owns the plan and keeps the physician informed about how the person is doing at home. The navigator owns the calendar: appointments, rides, and the extra hours of in-home support a family may need in the first days.
Wellnet works with the rest of Indwell. A home safety survey shows what the house needs. Wellfit rebuilds strength and balance. Wellfed brings meals. Wellmind follows up on thinking and mood. Wellnet keeps all of it moving in the right order.
